This was the penultimate issue of the Brainiac Story and was recalled by DC Comics for cover content. It depicts Superman, in civilian attire with his 'S' shield clearly visible, and his adoptive father outside the Kent farmhouse sharing a beer (or maybe a root beer). DC issued a statement to retailers that the issue was recalled, and that any copies featuring the original cover be destroyed.
The next week, DC reprinted the issue featuring a cover in which the label on the bottle was changed to read, "soda pop".

This was a mature comic where street thugs use obscene language but DC didn't want this language to actually appear so the text was placed in the speech bubbles and then blacked-out. However, in this issue slightly different shades of ink were used in the two elements leaving the vulgar words just readable and DC red faced.
This is the standard copy (there is also a 1:10 cover variant).

This is a collected edition, reprinting "Legion of the Damned" from Legionnaires #78-79 and Legion of Super-Heroes #123-124, featuring art by Olivier Coipel. However, the initial print confused many readers as it was published with the chapters out of sequence and was quickly subject to a recall/destroy notice (see picture).
You can easily spot the recalled issue as it has a yellow Legion logo (the replacement copies sported a white Legion logo)

In this issue Elektra appears nude, in shadow, in some fairly tame images. However, these were enough to prompt Marvel to recall the issue and give Elektra some underwear!
This one is signed by the cover artist Greg Horn in May 2004.

In Kyle Baker and Liz Glass' story, "Letitia Lerner, Superman's Babysitter," the super-toddler gets up to all sorts of shenanigans, including climbing into a microwave! When Paul Levitz, the president of DC Comics, objected to the portrayal of the little Clark Kent, most issues were recalled and pulped.
Ironically, the offending story won an Eisner Award and was subsequently reprinted in DC's Bizarro Comics collection.
This one escaped on a plane to Blighty but was then captured and encased in plastic.

This is the standard edition which a print run of around 7,000 and it is assumed that they all made it into the public domain before any litigation was initiated (hence the recall notice probably came too late).

This is a first print with the infamous “Marvel Douche” advert. Around 200 are thought to exist, escaping in preview packs and also on planes to Europe before the dreaded recall notice. The advert was an authentic (not fake as suggested elsewhere) advertisement for a Marvel-brand douche from the early part of the 20th century. Once spotted, DC Comics executive Paul Levitz ordered the entire print run destroyed over concerns that this could lead to litigation with their main rival (Marvel Comics). Notably the writer, Alan Moore, went on to create a "Miracle Douche Recall" headline on a newspaper in the humorous comic Top 10 linking the LOEG recall with the litigious Miracleman. Amazingly, not only was the Marvel-brand douche advert real, but the products themselves can still be seen today (see photo!). The second print replaced “Marvel” with the less controversial “Amaze”.

This comic was a DC reprint of the original first issue of Mad, however, inside DC left out the E.C. copyright notice from the reference to the original publication as well as mistakenly associating the characters and indicia to themselves instead of E.C. In addition to these DC also missed out both Gaines and Kurtman from the credits on the inside back cover. DC immediately withdrew the comic to correct all these problems.
The second picture shows the inside back cover credits from the recalled edition, with the corrected edition to the right for comparison.

Just after its release in early 2011, DC contacted retailers to say there was a printing error in Scooby Doo Where are You! #7 and that all copies should be destroyed with replacement being sent a couple of weeks later.
The recalled/error copy has an advert from 2009 for Absolute V for Vendetta, which does not fit the age profile of the readers of Scooby Doo, and is also a couple of years out of date.

The initial printing of this comic book was produced on the wrong paper stock giving it a matte finish. The comic was withdrawn and the cover reprinted using the correct glossy paper stock.
This one was mislabeled but has been corrected now in the registry database (thanks to Gemma for sorting that out).
